1.1Headquarter of Indian Railways is in Railway Board.Health Directorate of Railway Board controls and supervises Medical Department of Indian Railways.

1.2 Indian Railways are divided into 16 zonal Railways. Each zone is under administrative control of Chief Medical Director (CMD) of the Zonal Railways.

Aims&Objectives

Indian Railway Health Care delivery System has to perform following two important functions:

1.1 Functions related to Industrial Medicine

1.2 Functions related to Medical treatment to Railway beneficiaries

1.1Functions related to Industrial Medicine:

a) To attend Railway accident & other untoward incidences.

b) To provide emergency medical treatment to traveling sick passengers

c) Pre employment Medical examination to allow only fit & suitable candidates to join the services.

d) PME (Periodical Medical Examination) of serving employees to allow fit person to continue in those jobs which are related to safe running of the train.

e) Medical Boards & other medical certification of serving employees.

f) Control on loss of mandays on account of sickness (both normal & HOD). It is kept under control by medical officers.

g) Safe water supply at Railway Station:

A constant vigil is maintained to check on quality of drinking water made available at Railway Stations.

There is a well laid down system of reporting and to take corrective measures for unsatisfactory reports.

h) Safe food supply at Railway stations:

Following activities are undertaken to ensure safe food supply at Railway station.

a) Regular inspection of kitchen & food stalls.

b) Regular health check up of food handlers.

c) Collection of food samples under PFA Act.

d) Collection of food samples under quality control system.

i) Ensuring Factory Act:

Medical first aid posts are provided to all factories of Indian Railways (there are 84 such factories) as per Factory Act.

j) Workman Compensation Act:

Activities related to WCA is under taken on regular basis.

k) Certification of dead bodies at Railway Station, Railway Yards, Railway line, etc. is being done on regular basis.

l) Certification of perishable good in Railway Station about their disposal is done as and when required.

1.2 Functions related to Medical treatment to Railway beneficiaries:

Total comprehensive Medical treatment is provided by Indian Railway Medical Department.

Indian Railway health care delivery system is providing Industrial medicine as per the need of the Indian Railway and also providing medical treatment to Railway beneficiaries.

The system is a dynamic system which makes suitable changes in proper time to provide proper medical treatment to Railway beneficiaries.

Free medical treatment is provided to Railway employees & their dependant. At present we take average of 4 members per employee. Retired Railway employees who join RELHS scheme they along with their dependants are also entitled of free medical treatment. We take average 2 per member of RELHS

Health Care Delivery System

1.	Steps taken to improve the health care delivery system of Indian Railways
	1.1 Power given to Health Unit doctors situated 75 Kms away from Railway Hospital to get the routine Pathological . investigations like blood sugar, blood urea , S. Certainties, SGOT, SGPT, simple X-rays, etc. from outside. ECG is done by in house system.
	1.2 Engagement of Hony. Visiting Specialist : The present system of Hony. Consultants who are paid Rs. 5000/- pm for Central hospital & Rs. 4000/-pm for other Railway Hospitals will be gradually replaced by Hony. Visiting specialists. In total 575 such specialists can be engaged all over Indian Railways. They will be paid Rs. 20,000/-pm /Rs. 14,000/-pm and Rs. 7000/- pm for coming to Railway Hospital 6days/4 days and 2 days per week respectively. This is expected to provide adequate cover of specialist services in Railway Hospitals wherever our own specialist doctors are not available.
	1.3 To call specialist doctors from private sector for having consultations and for surgical operations on case to case basis. They will be paid for their professional services This facility will help Railway Hospital authority to provide adequate Specialist treatment in their Hospitals. 1.4. To make anesthetic available ,their rate if payment for their professional services has been increased. For Railway Hospitals with number of doctors post 9 or below anesthetic can be called without having a regular post of anesthetic in that Railway Hospital. 1.5 To provide better Ayurvedic & Homeopathic treatment in Railway Hospitals following important changes done:: Revised ceiling on Expenditure for Ayurvedic and Homeopathic doctors Earlier Revised a) Monthly honorarium for Homoeopathic/ Ayurvedic Doctors For 8 hours duration Rs. 10,000/-p.m. Rs. 15,000/-p.m. For 4 Hours duration Rs. 7,000/-p.m. Rs. 9,000/-p.m. b) Monthly honorarium for Ayurvedic Dispensers Rs. 5,000/-p.m. c) Monthly subsidy on medicines Earlier Revised (i) Ayurvedic Dispensaries Rs. 6,000/-p.m. Rs. 8,000/-p.m. (ii) Homoeopathic Dispensaries Rs. 2,000/-p.m. Rs. 4,000/-p.m. (includes Rs. 1,000/ p.m. for dispenser, if required) ( Ref:- E(W) 2008/WE-1/10 dated 7^th January 2009) 1.6 Revised monthly remuneration payable to Contract Medical Practitioners(CMPs) engaged on Railways as under :- 1.6.1 Category of Medical Full Time Part Time Practioners/Dental surgeons On contract basis. 4 Hours 2 Hours GDMO Rs. 39,400/- Rs. 19,400/- Rs. 9,700/- Dental Surgeon Rs. 39,400/- Rs. 19,400/- Rs. 9,700/- Specialist Rs. 46,800/- Rs. 22,900/- Rs. 11,500/- Super Specialist Rs. 53,900/- Rs. 26,300/- Rs. 13,200/- 1.6.2 The remuneration specified above includes the following amount as HRA and Transport Allowance in case of full time contract doctors: HRA Transport Allowance General Duty Medical Practitioners Rs. 5,250/- Rs. 3,712/- Specialists Rs. 6838/- Rs. 3,712/- Super Specialists Rs. 7375/- Rs. 3,712/- 1.6.3 In case of fulltime contract doctors for whom Railway accommodation is provided, an amount equivalent to the sum of HRA ( as indicated above)and license fee of the accommodation so provided be deducted from the monthly remuneration of the concerned medical practitioner. 1.6.4. For Senior Citizen of Railways: Pilot Project for issuance of Smart Card to RELHS beneficiaries in New Delhi/Delhi ( NCR area) for Cashless services to take treatment in recognized Private Hospitals of NCR area in Emergency
